Oregon Health Plan Members

I’m enrolled as a behavioral health provider with the Oregon Health Authority to provide and bill for services to OHP members with an “open card” (not assigned to Columbia Pacific or other CCO). I am not able to bill to Columbia Pacific CCO for behavioral health services for OHP members.

However, I can provide services to you when you change to an OHP open card. OHP members have the right to request changing their Columbia Pacific card to an open card to access needed services. If you want to pursue this option in order to access services at Clark Miller Therapy, first affirm that the medical providers and services you are using and want to use accept OHP open card. If that works out for you, you would call to request the change to an open card: 1-800-393-9855.

Health systems in the region that DO accept OHP open card for primary care and other medical services include: Adventist Health; Tillamook County health clinic; Providence Health; Rinehart Clinic in Wheeler. Contact me with any questions: 971-324-0061.
